A cross-enrollee is a regular student of a school (referred to as their home school), but who concurrently enrolls for a subject/subjects at HCDC (referred to as the host school) Must have completed requirements, such as: 1. Cross-Enrollment Permit 2. Certificate of Good Moral Character 3. Two (2) copies 1.5″ x 1.5″ ID pictures

1. Secure enrollment form from the Graduate School Office and do the following: a. Fill-out the form with the necessary information b. Secure approval from the Dean for subjects to be taken for the semester c. Submit 2 pcs. 2×2 ID picture (colored) and the recommendation letter from immediate head/former professor (Doctoral Students) 2. Proceed to the Office of Student Affairs (OSA) to obtain the student ID number. 3. Pay for the necessary fees at the Teller. 4. Go to Registrar’s Office for the: a. Encoding of subjects and releasing of class schedule b. Submission of the following: Certificate of Honorable Dismissal (CHD) from last school/institution attended or transfer credential (if student is a transferee) Special Order (S.O.) Official Transcript of Record (TOR) Authenticated Birth Certificate from Philippine Statistics Authority (PSA) Authenticated Marriage Contract (if married) from Philippine Statistics Authority (PSA) 1 pc 1×1 ID picture 5. Proceed to the Printshop for ID processing. 6. Take the Physical Examination at the Clinic.

Pre-Admission Procedure 1. Proceed to the Graduate School Office for the following: a. Pre-evaluation/Academic Assessment by the Dean or staff b. Interview with the Dean c. Releasing of Application for Entrance Examination 2. Pay for the Testing Fee at the Finance Office. 3. Submit the receipt and Entrance Examination application to the Guidance office for the acquisition of the examination schedule. 4. After taking the examination, procure a certification from the Guidance Office to be forwarded to the attending Graduate School staff.
